 New Patients at Seattle Naturopathic Center

Thank you for your interest. To become a new patient please:
  • Step One: Phone/email 206-319-5322 to book an appointment.
  • Step Two: Print of the new patient package and please take the time to fill them in prior to your first visit.
  • Step Three: Bring the completed forms to your first visit.

Patient Consultations


Patient consultations are scheduled on the basis of time needed to complete the planned activities. First consultation is scheduled for 45 minutes. Follow-up consultations are scheduled for 30 minutes for a regular visit, and 60 minutes for extended consultations (such as complete physical exams). Medical procedures may be priced separately from consultations.

Cancellation of Appointments

Appointments may be cancelled with no charge any time up to 24 hours before the time of the appointment. Appointments missed without notice will be billed to the patient at one half of the cost of the scheduled appointment.


Insurance

Dr. Diane Lee is contracted with most major health insurance companies, including Regence, Premera, Aetna, Cigna, Lifewise, First Choice, Great-West Healthcare, Health Allies, Access One Consumer Health, Medical Resources, PacifiCare Health Systems, and United Healthcare. Many out of state insurance companies also cover naturopathic primary care service. Most plans have "out of network" coverage if the doctor is not credentialed with you specific insurance company. Call to ask about your coverage!

Payment Policy

Payment is due at the time of the patient consultation, when laboratory tests are ordered, and when prescribed medicines are delivered. Payment for special order medicines is due when they are ordered.
